Golden Achievement Awards
BALLOT
SPECIAL VOTING NOTES:
- Voting is from Feb. 1st - Mar. 15th
- Only ONE vote per valid email address and TWO votes per IP address will be accepted.
- For each individual category, for which you choose to vote, all lines must be completed with different entries in order for your vote to count.
- All Venues must have city listed. (e.g.: Karaoke Klub, Ventura)
- North LA County consists of San Fernando & Santa Clarita Valleys, Lancaster, and Palmdale.
- First and Last Names of All entries must be included. (There are many people with the same First or Last name!)
- Do not enter "Same As Above" in the ballot. As we are unable to determine to whom this refers!
- Do not use any non-alphanumeric symbols [e.g. - ' " ? ( ) , : ; @ # $ % & *] in the ballot.
- You may not vote for the same person more than once in any individual category or your entire ballot will not be counted.
- Last year's winners are not eligible for selection in the same category for one year. However, they are eligible in other categories.
- Associates of Karaoke Scene Magazine are NOT eligible for awards; these include, but are not limited to:
(Peter Parker, Manny Pacheco, Yvonne Erman, Sammy Yamamoto, Cassidy, Tony DeFazio, Candace Klewer, Kool Karl, Jessy Lee, Fran Emmons, Brigitte Wolf, Shari Shefflet, Michael Valintino, Debbie Smith, and Greg Boone.)
